Radiologist – Submit Most Recent X-rays labelled and dated (6 views 2 Sites Max)
Cardiologist – Med Dosages, Recent lab work, X-rays (only review cardiac structures)
Criticalist – Medical history, Med Dosages, lab work & X-ray reports + (detailed online forms)
Internal Med. – Medical History, Med Dosages, lab work & X-ray reports + (detailed online forms)

Online forms lacking information will not be sent to Internal med, or criticalist.
Information is vital for these submissions. Cases will be held until more information is recieved.
