Submit ONLY most recent (Lab work, Rad Reports)
Dr. LU reports rely on up to date information\/ results
combined with her ultrasound findings.

Small: 1-29 lbs \/ Medium: 30-49 lbs \/ Large: + 50 lbs

Radiologist – Submit Most Recent X-rays labelled and dated (6 views 2 Sites Max)

Cardiologist & Internal Med \/ Criticalist – Medical History, Med Dosages, lab work & X-ray reports + (detailed online forms)

Online forms lacking information will not be sent to Internal med, or criticalist.
Information is vital for these submissions. Cases will be held until more information is recieved.
